Stable, client-facing error taxonomy for the intake HTTP surface.

Each code carries the HTTP status it renders as and a curated, safe default message. The wire contract is the code plus the status — never an upstream or internal exception message. Codes are stable identifiers clients can branch on; the human-readable messages can be reworded without breaking that contract.
